This is a care management position, in which Care Manager – RNs will create and administer care plans, rather than rendering direct clinical services. QUALIFICATIONS
We are seeking Nurse Practitioners to help us perform annual clinical assessments to members in the comfort of their homes. You will be responsible for conducting in-home assessments including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam with 15 in home visits per week.
This position is Monday - Friday 8:30am - 4:30pm, no weekends or holidays!
